We will keep your plans effective date on file with our office. You can sign up for auto-renewal and save 5% on next year’s premium! If you do not choose to auto-renew and your plan lapses, a $75 reinstatement fee will be applied to your account.
GUIDELINES
- This is a discount plan, not an insurance plan, and cannot be used at any other office.
- Our plan cannot be used in conjunction with any other dental plan or insurance.
- No refund will be issued if the member decides not to use the plan.
- The patient’s portion of the bill is due at time of service.
